This exact thing just happened to me. Made it all the way to the very end of the level and was forced to go back to the Cosmodrome, where I’m now trapped. Lost hours of my life on this game.
I did the cosmodrome level early and then found posts saying dont do this. The game would only allow me restart at the very start of Act 3 but I did this to avoid any future problems. I have to redo cosmodrome but hopefully all will be OK
The organisation of collectables in chapters in Act 3 are a bit strange - not really open world
I did not like the difficulty spike at end of Act 2 - I had to drop to Beginner to finish the Act
I dont understand Relic weapons - I can collect them once but then they disappear. Why is Gears inventory so weird?
There was a difficulty spike? I only had an issue with the matriarch because it kept snapping out of the water and killing me instantly cause of the jump. And what do you mean it confusing? They’re powerful variants of the other weapons so of course its a one time only thing. Unless you drop them the only time you lose them is at the end of act 3.
Yeah, I just gave up looking for solutions and just started a new save from the start of Act 3. I basically did a speedrun and skipped side stuff until I stopped seeing the desert, and by the time I had gotten to the “epic” climax of Act 3, I was so burned out I couldn’t even enjoy it.
I’ve decided to take a few days break from Gears 5 for a few days before I start Act 4.

Likewise on a break. No need to rush in just having Game Pass - the old way of waiting before committing time to a new game is better.
The game still broken, I played both Xbox One and PC version and I the game freezes and crashes. Recently on Steam version I bought yesterday I got BSOD, this is what BlueScreenViewer shows:
At least Nvidia replied me, but TC must be aware of this problem and work together with Nvidia to fix It.
On Xbox One the game suddenly closes Itself, so I’m not the problem or my PC, the game is broken, no matter if is on PC or Xbox One.
I haven’t gone back to it in 2 weeks now.
I have been playing the campaign on insane in coop and in act 2 the game has just stopped saving progress at all and its undone like 5 hours of progress then we ran into a glitch where my partner couldnt equip a weapon making us have to restart
A month after your post, the same happened to me! Win10 Store version, Windows 10 1903.
I can’t pick up upgrade modules in the desert, in pump station side mission? What the ■■■■ ? And in the water tower? Driving me mad
Update:
In the past 5 days, every time I tried to start the game, it put me through initialization - brightness, safe zones, Boot Camp, et cetra. It does NOT have my profile or game save. When I quit from boot camp, there’s a 50% chance of GW502.
This shouldn’t be what a normal game or a normal cloud service does.

Ok so Im on act 1 Shot in the dark, and I know Dave is supposed to fly through this little hole to open the door. Nope the game is still playing music like enemies are in the area (killed them all) so it won’t let Dave open the dang door.
Same to me, i don’t have any courage to restart the chapter 
Another thing to add to the list is that the components don’t fully refresh/respawn on the world map when u hit a certain part of the game. I had to restart Act 3 multiple times (got 60 on first run, 80 on next , and 147 on the third) …just lets us keep the components
I want to add this to my current thread, this one is kind of ancient.
Glad I beat the campaign as soon as I got the game. Just before the issues started I got to the final chapter. Sorry you guys are having issues.

The only thing stopping me from my seriously achievement is the campaign act 1 chapter 2 will not unlock on my stats for completing it I have joined co-op and hosted co-op but never shows I completed that chapter. I wouldn’t mind but it’s the shortest easiest chapter in the whole campaign .it doesn’t even show that I completed that particular chapter on any difficulty. Any suggestions?
Played through section II, way past the lab level. This game just sent me back to a seemingly random save point, after you find the skiff but it’s before you start on the skiff and that’s multiple HOURS of gameplay lost. WHen I reached out to “Gears of War Support” about the issue, they responded ONCE and then IGNORED ME. This is what they said:
